Supervision
- Are students supervised at all times?
- Have adults who supervise students been fingerprinted?
Adequate Training
- What training will be necessary to complete the project well and safely?
- Who is best able to provide that training?
- How can we best prepare students with the specific skills they need to complete
their work?
Equipment
- What equipment is necessary for all students to be actively engaged in the
project?
- Who will provide this equipment?
Medical Care/Appropriate Dress
- Who will have a first aid kit and who will have medical releases in case
of emergency?
- Are on site supervisors informed of any medical allergies or concerns?
- Do on site supervisors have access to medical release forms in case of an emergency?
- Do students need any special clothing?
Overall Safety
- Are there any safety issues related to transportation that we need to prepare for?
- Is a cell phone needed in case of an emergency? If so, who will provide it?
- If the participant is under 18, have you provided parents with an accurate description of the project and have they signed a release indicating that they know of any risks involved and authorizing their child's participation?
